Q: Does it come with screws/bolts for mounting?

I would say NO, you should bolt them into solid wood or beams under decking. When someone leans back you can see the deck move. So secure very well. Not everyone treats your boat like you do, so if you get the guest that puts there feet up and leans back on the seat, they must be secure.

Q: Does this mount swivel?

The mount adjusts for height but does not swivel. We added a small swivel (separate piece) on top of the mount, then attached the seat to that...now the seat swivels very nicely. We ordered the swivel on Amazon as well, and it was only about $7. Very easy to mount. Recommemd highly!

Q: Can you drill another set of holes so the lowest height is shorter than 11.5"?

You can drill all the holes you want, but that isn't going to make the product any shorter.
